In order to withdraw by credit/debit card, simply go to My Account → Transactions → Withdrawal.
There, you'll have the choice of the card(s) that you've used to deposit.
Please note that you may be asked to verify your withdrawal. For more information regarding withdrawal verification via card, please go to My Account → Profile → Verification process.
